Detecting New Issues

Do not wait to be “updated” in a status meeting that there is an issue.
Issues need to trigger discussion and then action with little delay.
An issue is usually one of:
A metric is out of its expected range.
Something external occurs that changes assumptions made in a prior decision.
A new idea is proposed.
Conflict or dysfunction becomes apparent.
This event-based approach requires that,
There have been quality discussions about the current model of how the value stream works.
Metrics about the above have been defined.
People (including managers) are paying attention to the metrics.
People are not afraid to notify managers when an issue arises.
