Detecting New Issues

Do not wait to be “updated” in a status meeting that there is an issue.
Issues need to trigger discussion and then action with little delay.
An issue is usually one of:
- A metric is out of its expected range.
- Something external occurs that changes assumptions made in a prior decision.
- A new idea is proposed.
- Conflict or dysfunction becomes apparent.
This event-based approach requires that,
- There have been quality discussions about the current model of how the value stream works.
- Metrics about the above have been defined.
- People (including managers) are paying attention to the metrics.
- People are not afraid to notify managers when an issue arises.
